WHAT IS AN ACO?

An ACO is a group of doctors, hospitals, and other health care providers, who come together voluntarily to give coordinated high quality care to the patients they serve.

Coordinated care helps ensure that patients, especially the chronically ill, get the right care at the right time, with the goal of avoiding unnecessary duplication of services and preventing medical errors.

How ACOs Work

The goal is to ensure patients receive the right care at the right time, while avoiding unnecessary services and medical errors.

If an ACO succeeds in both improving care quality and reducing healthcare costs compared to established benchmarks, it shares in the savings generated—creating financial incentives for value-based care. Performance is tracked through quality metrics, patient outcomes, and cost data.

Our providers receive an Achieved Savings Bonus by contributing to the ACO’s ability to reduce overall healthcare costs while maintaining or improving quality of care.

Actively contributing to cost reduction and quality improvement, not only helps the ACO meet its financial and clinical goals, but also saves lives through preventive care.

STEP 1: JOIN ACO

The provider signs a participation agreement with the ACO, aligning themselves with the group’s care coordination, quality reporting, and cost-saving initiatives.



STEP 2: DELIVER CARE

By focusing on preventive care, reducing unnecessary services, closing care gaps, and managing chronic conditions effectively, the provider helps improve outcomes and reduce total costs for attributed patients.



STEP 3: YEARLY EVALUATION

At the end of the performance year, CMS (or another payer) compares the actual cost of care for the ACO’s patient population to a pre-set benchmark. If the ACO’s total spending is below the benchmark and meets quality performance metrics, it generates “shared savings.”
